Surgical Approach
Retrosigmoid (Suboccipital)
A lateral suboccipital craniotomy just behind the sigmoid sinus that opens the cerebellopontine angle, with hearing preservation possible. It is the standard route for vestibular schwannomas, CPA meningiomas and microvascular decompression for trigeminal neuralgia and hemifacial spasm.
Cranial-nerve monitoring and gentle cerebellar relaxation through CSF drainage are central to a safe corridor.
