Surgical Approach
Pterional
The workhorse frontotemporal craniotomy centred on the pterion, splitting the Sylvian fissure to reach the anterior circulation, the suprasellar and parasellar cisterns and the medial sphenoid wing. Wide fissure dissection and proximal vascular control make it the default for anterior-circulation aneurysms and many basal tumours.
Its versatility and low profile make it one of the most frequently used cranial approaches.
