Surgical Approach
Endoscopic Endonasal Transsphenoidal
A minimally invasive route through the nostrils and sphenoid sinus to the midline skull base, using the endoscope for a panoramic, close-up view. It is the standard approach for pituitary adenomas and, in extended variants, reaches the tuberculum, planum, clivus and odontoid.
Its advantages are the absence of brain retraction and direct decompression of the optic chiasm; the principal challenge is watertight skull-base reconstruction to prevent a CSF leak.
