Pathology
Dural AV Fistula
Dural arteriovenous fistulas are abnormal shunts within the dura between meningeal arteries and venous sinuses or cortical veins. Risk is stratified by venous drainage: cortical venous reflux (Borden II–III, Cognard IIb–V) carries a substantial haemorrhage and neurological risk.
Treatment is obliteration of the fistulous point, usually by endovascular embolisation, with surgical disconnection of the draining vein when endovascular access fails.
